Our practice – your rights and privacy
The protection of privacy of our patients using our services at Oak Tree Osteopathy is very important to us. As such we have implemented policies and procedures to ensure this.
Osteopaths must keep clinical records about and relating to your healthcare. It is a government requirement, and information about your medical and family health history is needed to provide accurate diagnoses and appropriate treatment.
By providing Oak Tree Osteopathy with information and using the services provided, you consent to the collection, use and disclosure of information in the manner described in this Privacy Policy.
Information we may collect:
The following information may be collected by Oak Tree Osteopathy:
A patient’s name, contact details (including postal address, telephone, fax & mobile numbers, and email addresses) and date of birth
Credit Card or bank account details of users wishing to pay their account via credit card or direct debit
Oak Tree Osteopathy collects information from users where information is:
Entered by users online via our website (Cliniko online booking);
Faxed or emailed to our clinic;
Sent via conventional post; and
Collected over the telephone.
Except as outlined in this Privacy Policy:
Oak Tree Osteopathy is the sole user of the information collected from our clients; and
We do not use, share, sell or rent this information in any way.
Information collected from third parties:
All patients are generally asked for consent for records transfers and are informed of the reason and purpose for the transfer. Sometimes, a legal order for records can be made by third parties like tribunals, courts, work cover schemes and private health funds. If made, we let you know as early as possible and keep you informed.
Where patient data may be stored:
Clinical records of all patients are kept in an encrypted electronic system that acts as secure storage. It is necessary for us to keep your health information for as long as is required by law (generally 7 years after your last consultation, but longer in some circumstances). We will inform you if we intend to transfer your clinical records to another osteopath or clinic, or if we sell or close this clinic.
We use Cliniko to enable online bookings and to store your medical records electronically. Access to medical records:
You have the right to access your information. You may ask to view the information or ask for a copy of a part or of the whole record (a cost may be involved in accordance with Australia’s Privacy Guidelines.
